Doctor loan monthly payment calculation:

1. Loan Amount = Home Price - Down Payment
2. No PMI (waived for loans up to $1M, LTV <= 100%)
3. Monthly Payment = M = P x [r(1+r)^n] / [(1+r)^n - 1]
   where P = loan amount, r = monthly rate, n = months
